Baldness or Alopecia affects the population worldwide. Hair loss can be the reason of individual losing the self esteem. Hair transplant is solution for alopecia and leading doctors all over the world recommending the Follicular unit extraction (FUE).

FUE is a method of graft harvest whereby punches of various types are used to remove the follicular units from the donor region one at a time. To extract the follicular unit the surgeon creates a site and then implants the graft. For harvesting hair is trimmed to have a complete visualization of hair entry into scalp. The usual site for taking hair is back of the head also called as occipital region. For taking the follicular unit patient is made to lie in prone position and surgeon sits behind and slightly below the patient. The donor area is anesthetized with local anesthesia solution. FUE is performed under 2.5X to 6X magnification with good lighting source. With the experienced hand punch size can vary from 0.6mm to 1mm. The device for punch is aligned the direction of hair bulb. The hair follicular unit is extracted manually with the help of forceps. The extracted hair stored in proper solutions at colder temperatures.

Pre-made slits or Incision

In this method, incisions are pre-made on the scalp with small blades or needles in the affected balding areas of the scalp which is also called as the recipient area. While the FUE surgeon is making incisions, the follicular units previously harvested lie in a standard storage solution to prevent dehydration and also prevent any damage to the follicles. Follicular units are then carefully placed into recipient sites by the trained technicians. The Surgeon may chose to inspect FUE graft further under microscopic dissection before its implantation on the scalp.

5 steps in pre made slits or incision

  • The surgeon first open the hole with the forceps superficially till the epidermis only. If it goes deep then bleeding might occur and the depth is measured by measuring the length of graft
  • A needle is placed in the anterior border of the slit to identify which slit was opened.
  • The graft is held at the level of fat below graft in Strip & at the level of the bulb in FUE
  • Slide then it is hole with the help of forceps
  • Again the needle is used to stabilize it, otherwise kinking may occur

Stick and Place: Simultaneous Site Creation and Implantation

In this method, site creation and graft implantation are performed simultaneously as part of a one or two step process.

Two Step “Stick and Place” Technique:

In this process qualified surgeon makes a recipient site incision with a needle and then immediately places the follicular unit graft into the incised scalp using forceps.

One Step “Stick and Place” Technique:

In this one step technique the trained surgeon uses a device, called an “implanter pen”. With the help of implanter pen surgeon make both a recipient site and implant the follicular unit almost simultaneously. It is usually called as one step technique but the surgeon is required to pre-load each implanter pen before the implantation. It may take some extra time and required additional experience and skill.

It is an instrument used by physician to transfer FUE graft into scalp. An “implanter pen” is a pen-like device featuring a hollow needle attached to a tube-plunger apparatus It reduces the time of follicular unit spend time outside the body. It reduces the direct contact with patient scalp. In FUE surgery surgeon prepare a graft and then technician load the follicle with special needle to implanter Operating surgeon presses the plunger and inject the graft through the needle into the scalp. Angling the needle to proper angle is important to ensure the natural growth of hair. The main advantage of these devices is that it minimizes the bleeding and the chance of infection. It also reduces the scarring. As surgeon put the hair follicle immediately the incision has very less time of bleeding. The hair has better survivability with implanter pen because surgeon is not touching vital structure like dermal papillae in the hair root. The Choi implanter pen is more suited to Asian as compared to other races because Asian hair shaft diameter is generally thick and straight so they are easily loaded in implanter. The other races may have curly hair which can’t be loaded properly in implanter pen.

The Lion Implanter Pen by Hans Biomedical

It is a modified version of choi implanter pen. It features a “clip” that allows for easier and faster needle replacement. This reduces surgical downtime and heartens the technicians to change needles when necessary. Needle dulling can cause scalp trauma, so the ability to change needles quickly and easily helps promote effective and fast needle substitution when needed. The needles are sharper and sterlized with ethylene oxide gas, so less chances of infection.

SAVA Implanter

The surgeon usually complaint that choi implanter is costly and has many moving part, which can breakdown during surgery. To counter this problem Indian surgeon Dr Vasu designed the SAVA Implanter. It is cost effective and consists of transplanter and an inducer. The graft is placed in transplanter and graft is slide in using inducer.

KEEP Implanter

It is the newest implanter and it can be used by two people simultaneously. It does not require technician to the load the graft, surgeon can do both steps himself. Keep the graft in non dominant hand and roll the graft to implanter with dominant hand and once loaded to implant it himself.

Machine Assisted Implanter

Follicular units below the skin is always a puzzle for surgeons. The angle of hair changes as it leaves the scalp. The surgeons can only estimate it. Hence, advancement of punch inside the scalp is always a game of chance that leads to transection of grafts. To minimise the transection rate and enhance the speed of FUE, a French company MEDICAMAT has created a device called as SAFER®/NeoGraft® machine to automate FUE transplant. It helps in reduction of standard operation time as well as the transaction rate³.

The grafts which are extracted can be taken out from the receiver flask and arranged on the gauze. These grafts are then sucked into the hand, piece by suction pressure and implanted.

Disadvantage

  • The machine is expensive.
  • Although the company claims less trauma of grafts, but the biggest drawback with this machine is that suction may damage (as per the off hand information by some surgeons) the follicular unit grafts.

Robotic System

This is a technique developed by Restoration Robotics San Jose CA. A robotic system ARTAS has been developed to operate under physician control. It use digital video guided microscope and follicular unit identification system with two step punch device with a precision of 0.014mm. Since the robot is doing the work, the surgeon fatigue is very less. It is very expensive and has limited availability in the world.

Pharmacokinetics

Betamethasone is glucocorticoid receptor agonist. It leads to control the rate of protein synthesis, depresses the migration of polymorphonuclear leucocytes & fibroblasts; reverses capillary permeability and helps in…. lysosomal stabilization at the cellular level to prevent and control the inflammation. When taken orally, half life of the drug is 6.5 hrs; it is metabolized by the liver and excreted through urine.  

USES

  1. Betamethasone is used on the skin to relieve the redness, swelling, itching and discomfort of many skin problems such as: psoriasis eczema and various types of dermatitis.
  2. Betamethasone Lotion is also used on the scalp and hairy parts of the body.
  3. It is also used in various type of auto immune disorder as well as various type of malignancy.

Your doctor may have prescribed Betamethasone forother reasons also.

Long term use of over dosage of betamethasone is not expected to produce life threatening symptoms. However, prolong useof this highly potent steroid can lead to symptoms such as skin thinning, bruising, redistribution of body fat (especially in your face, neck, back, and waist), flaring of acne or hair on face, irregular menstruation, impotent, or loss of interest in libido,etc.

Betamethasone and Alcohol

Both alcohol and betamethasone topical can dry the skin, and the combination may dry the skin even more. Drinking alcohol should be avoided while taking betamethasone in any form.

CONTRAINDICATION

Do not use Betamethasone if you have had an allergic reaction, such as wheezing, rash or hives, to betamethasone

Also do not use betamethasone if you have:

  • A viral skin infection, such as cold sores,
  • The fungal skin infection, such as tinea or ringworm, thrush
  • Tuberculosis of the skin
  • Acne rosacea
  • Inflammation around the mouth

Betamethasone topical comes in a 0.05 percent ointment or lotion.

Whenever betamethasone is applied topically on any body part, it should be exposed to the air. Do not cover it with bandage. Do not apply the cream on face, underarms, and groins.  Please consult your doctor before applying cream to above mentioned areas. Do not apply more than 50gm in a week. Stop the betamethasone when the existing skin condition subsided.

While most people might think that bunny lines sound cute, the fact of the matter is that bunny lines can be source of woe for many people. For years, laugh lines and crow’s feet were what troubled most people, particularly women, its bunny lines these days that are causing sleepless nights for many. What are bunny lines?

Have you ever noticed those fine lines that appear when your nose gets all scrunched up, because of a laugh or a deep smile? These are the lines that are known as bunny lines and while these are normally age associated, these could also be tell-tale signs that you have had Botox injections in the past.   • Botox – When you get Botox injections, they are relaxing certain muscles of your face, but when these muscles relax, there are certain other muscles that have work a lot more. However, there could be a little discomfort from when the fine needle is being inserted into the muscles. When Botox has been administered in the right dose and in the proper manner, there is no reason why anyone should be able to identify that you have had Botox.

With the help of a microneedle, anywhere between one to three doses will be administered and each will happen one after the other. In most cases, there is only a stinging or burning sensation, but the pain is minimal, because of the size of the needle. What is Dutasteride?

Dutasteride is a 5-alpha-reductase inhibitor drug similar to the finasteride. Dutasteride or Avodart is a dual inhibitor of both type I & type II 5-alpha-reductases. It inhibits the conversion of testosterone to dihydrotestosterone which is a key mediator of male pattern hair loss. Dutasteride is commercially available as ‘Avodart’ and is manufactured by the Glaxo-Smith-Kline Pharmaceuticals.

Originally dutasteride 0.5mg or Avodart 0.5mg is approved for the treatment of the BPH (symptomatic benign prostate hyperplasia. Dutasteride is not approved by FDA for hair loss but it is available as ‘off-label’ hair loss treatment for men at a dose of 0.5mg. However, Dutasteride is approved for hair loss in South Korea after clinical trials on Korean population. It is also approved in US, Japan and is not approved in Europe yet.

Why Dutasteride for hair loss should be used?

Scientific researchers have discovered that men who are suffering from either male pattern hair loss or benign prostatic hyperplasia, have an increased level of the DHT (dihydrotestosterone) in their body. The hormone testosterone is converted into the dihydrotestosterone after the enzymatic action of 5-alpha reductase. DHT causes the hair follicles to shrink resulting in hair fall. Dutasteride acts on the DHT hormone and promotes the hair growth by increasing the number of hair follicles.

How Dutasteride stops hair loss?

Dutasteride is the first dual inhibitor of the reductase enzymes. Its dual mechanism of action results in the DHT synthesis and catalytic activity of 5-alpha reductase type 1&2 enzymes. This dual action leads to the complete suppression of the serum DHT levels more than 90%. When this transmission of DHT to the hair follicles is stopped, then there is no miniaturization of the hair follicles. When there is no transmission of the DHT to the hair follicles, this results in the elongated anagen phase (growing phase) and faster hair growth.

Dutasteride Vs. Finasteride

Both Finasteride & Dutasteride are 5 alpha reductase inhibitors but dutasteride reduces the larger amount of dihydrotestosterone levels than finasteride. Less DHT levels mean more chances of reduction in hair loss but the reduction in the DHT levels can cause other hormonal imbalances or physiological changes in the body. Unlike finasteride, dutasteride prevents both types of 5 alpha reductase enzymes from undergoing a metabolic process. Finasteride is an old drug used for the treatment of male pattern baldness while dutasteride is relatively a new drug for hair loss and is still under clinical trials.

Clinical Trials on Dutasteride:

Dutasteride resulted in scalp and serum dihydrotestosterone levels to decrease but increases the testosterone levels in a dose dependent manner. Dutasteride can decreases serum DHT levels by more than 90% whereas finasteride can decreases serum DHT levels by 70%.

Medical Uses of Dutasteride:

  • Prostate enlargement: Dutasteride is primarily used for the treatment of benign prostatic hyperplasia (BPH) or also known as an “enlarged prostate”. Dutasteride has an approval from Food and Drug Administration (FDA) in the U.S. for the treatment of BPH.
  • Prostate cancer: According to a clinical review in 2010, a 25–26% reduction has been noticed in the risk of developing prostate cancer with 5α-reductase inhibitor chemoprevention.  Although there is insufficient data to determine if they have an effect on the overall risk of death from prostate cancer.
  • Scalp hair loss: In Japan and South Korea, Dutasteride has been approved for the treatment of male androgenetic alopecia (R). Dutasteride has also been used off-label for the treatment of female pattern hair loss.
  • Transgender women: Dutasteride is also used sometimes as one of the component of hormone therapy in transgender women. It is given in combination with an estrogen or other anti-androgens like spironolactone. It may also be used for treating the scalp hair loss or in people who can’t tolerate spironolactone.

Dosage of Dutasteride:

The off-label recommended dosage of dutasteride for hair loss if 0.5mg  per day. It has been clinically proven in some trials that dutasteride 0.5mg per day improves more hair growth in men to a greater extent than finasteride at 2.5mg per day.

Missed  Dosage:

If you miss dose of dutasteride, then no need to worry. You can skip this dosage and continue to your next dosage as recommended by the doctor. Do not take excessive dosage or double dose of dutasteride to balance your missed dose.

Overdose of Dutasteride:

Dutasteride Side Effects:

Interactions of Dutasteride:

5 alpha-reductase inhibitors like Dutasteride, can also stop the formation of the neurosteroid metabolites like allopregnanolone from progesterone. As a result of which it may reduce or even block the sedative, anticonvulsant, anxiolytic and different other effects of progesterone, particularly in the case of oral progesterone.

Contraindications of Dutasteride:

Women who are pregnant or are planning of pregnancy, should not take dutasteride. Dutasteride can result in birth defects, specifically ambiguous genitalia and under-masculinization, in male fetuses. This is because of its anti-androgenic activity and is mostly observed in the case of 5 alpha-reductase deficiency. If you are taking dutasteride, do not donate the blood at least for 6 months after your treatment has stopped. In the case of children or other people having hypersensitivity like serious skin reactions or angioedema, to dutasteride, should not take it.
